1. Each time a student enrolls you are required to complete a new Home Language Survey.  The link below takes you to OSPI's (Office of Superintendent of Public Instruction) website to print a form in a variety of languages. 

   Choose one of these options to get the completed form to the school:
        1) Complete, Save, and Attach the form to the online registration.
        2) Bring completed forms to your student's school.
        3) Forms are available at the school if you do not have a printer.
   
   Home Language Survey Form  (located the Home Language Surveys banner in light blue and expand the area to see the various forms) OR forms are available at any school.

2. Do you need materials in a language other than English? If so, we have documents translated into our top 5 foreign languages:
   • Arabic
   • Korean
   • Russian
   • Spanish
   • Vietnamese
