Fill out the questionnaire to determine what type of service is being provided and submit with the appropriate packet in Step 2.

Using the results from the questionnaire in Step 1 above, select the required packet and complete all requirements. Submit all paperwork to the person you are working with at the District. They will send the complete packet to the District office, attention Accounts Payable. Work should not commence until the complete packet has been turned in.
